Requirements
  • Graduation from an approved certification, program, accredited school, undergraduate school, graduate school, or postgraduate education sufficient to perform the job.
  • Hold a valid and unencumbered license or verification to practice in the state where services are provided.
  • Have the professional experience required by state regulations.
  • Successfully complete assigned screening and competency testing with a proficiency score of at least 80%.
  • Have reliable transportation.
  • Provide current tuberculosis results or a chest X-ray.
  • Provide current cardiopulmonary resuscitation certification according to agency policy.
  • Be flexible and organized and exercise independent, sound judgment.
  • Meet the physical requirements, including frequent sitting, standing, stopping, bending, kneeling, walking, balancing, crouching, crawling, pushing, pulling, reaching, and lifting or moving objects as required for safe client care.
  • Have vision and hearing within normal limits, with or without aid.
Responsibilities
  • Provide skilled nursing as ordered in the plan of treatment and nursing care plan.
  • Demonstrate and document clinical skills during each shift or visit in accordance with physician orders.
  • Identify new problems, changes, and resolutions and follow through with supervisory notification, obtaining necessary physician orders, and documenting them in clinical notes, supplemental physician orders, and the nursing care plan.
  • Observe aseptic technique as required.
  • Perform specific treatments and administer medications according to the care plan.
  • Collaborate as a team member with the patient, physician, and health care team.
  • Identify issues of concern and demonstrate critical thinking.
  • Provide skilled nursing care in compliance with state and federal regulations.
  • Perform related work as assigned.
  • Participate in agency-mandated in-service education and committee activities related to patient care.
  • Support and execute the company’s mission, ethics, and goals.
  • Complete clinical documentation accurately and submit it on time according to agency policy and guidelines.
  • Maintain a positive and professional presence in the company and community.
  • Follow the dress code and maintain a clean, neat professional appearance.
  • Report on time and as scheduled to complete work within the designated time.
  • Follow company policies and procedures.
  • Travel frequently to complete home health visits in private homes.
