Requirements
  • High school diploma or equivalent.
  • Certification issued by state where services will be performed: MD: Certified Nursing Assistant through the Maryland Department of Health; OH: Nurse's Aide through the Ohio Department of Health; PA: Nurse's Aide through the Pennsylvania Department of Health; WV: Nurse's Aide Certification through the West Virginia Department of Health and Human Resources.
  • Valid driver’s license.
  • CPR Certification required within 30 days of hire date.
Responsibilities
  • Provides personal and hygienic care every visit as per care plan and basic care standard in accordance with age-specific needs and policy and procedure.
  • Maintains a clean, safe, and comfortable environment per the basic care standard within the limitations of the home setting.
  • Ensures privacy and demonstrates concern for dignity of patient and respects patient’s property in performing all patient care.
  • Completes assignments in a timely manner as per standard.
  • Follows plan of care assigned by RN.
  • Relays critical information to the appropriate staff regarding patient condition.
  • Confers with the primary nurse on the original plan of care and any subsequent updates.
  • Documents patient status and care of services furnished in appropriate areas as defined by the documentation policy and procedure.
  • Reports observed changes in patient condition to R.N.’s, including recognition and reporting of changes in level of comfort.
  • Reports abnormalities in vital signs to R.N.
  • Utilizes supplies effectively and to ensure cost containment.
  • Perform the following treatments/procedures according to the specific standards including but not limited to: A. Measuring vital signs B. Lifting and moving patients C. Positioning the patient in bed D. Transferring patient from bed to chair E. Ambulating patient F. ROM G. Bed-making H. Personal Hygiene 1. Oral care – brushing teeth, denture care 2. Bathing – bed bath, tub bath, shower 3. Back massage 4. Perineal Care 5. Hair Care – brushing and combing, shampooing in tub or shower, at sink, in bed 6. Shaving 7. Nail Care 8. Changing clothes I. Assisting with elimination 1. Bed pan 2. Urinal 3. BSC 4. Ostomy care J. Patients’ weights K. Case care
  • Identifies the stages of death and dying.
