Requirements
  • A high school diploma or General Educational Development (GED) credential is required.
  • Must meet Center for Medicare and Medicaid Services-approved state and/or national certification requirements within the required state or CMS timeline.
  • Must possess all applicable state licensure, education, and training required for the role.
  • Must successfully complete the organization’s dialysis training program and CPR certification.
  • Must be able to perform direct patient care involving heavy lifting, patient movement, ambulation assistance, prolonged standing, bending, and occasional pushing or pulling of equipment.
  • Must pass the Ishihara Color Blindness Test as a condition of employment, subject to consideration of reasonable accommodations.
Responsibilities
  • Provide safe and effective dialysis therapy under the direct supervision of a licensed nurse in accordance with organizational policies, procedures, training, and applicable regulations.
  • Set up and operate hemodialysis machines and perform required pretreatment machine alarm testing.
  • Welcome assigned patients, assess their wellbeing, and evaluate patients before, during, and after dialysis treatment.
  • Evaluate vascular access for patency, perform vascular access cannulation, administer heparin when delegated or permitted by state law, and monitor vascular access during treatment.
  • Obtain and document pre- and post-treatment vital signs, weights, vascular access assessments, treatment parameters, and patient evaluations.
  • Initiate and discontinue dialysis treatments according to prescribed orders and established procedures.
  • Calculate and enter dialysis machine programming for ultrafiltration goals, treatment time, and ultrafiltration modeling as prescribed.
  • Monitor patients’ responses to dialysis therapy, evaluate intradialytic problems, and provide prescribed interventions.
  • Ensure appropriate safety alarms are enabled and conduct machine safety checks according to facility policy.
  • Report significant information, changes in patient condition, and unusual findings to the registered nurse or supervisor.
  • Obtain prescribed laboratory testing, collect and prepare specimens, label and store samples, and coordinate collection by the appropriate laboratory courier.
  • Clean and disinfect dialysis machines, chairs, equipment, and surrounding areas between treatments.
  • Maintain a safe, clean, and environmentally appropriate working area and immediately clean blood spills with approved disinfectant.
  • Prepare, organize, and efficiently use supplies and equipment to prevent wastage.
  • Monitor patients performing self-care under registered nurse supervision.
  • Assist in training applicable direct patient care staff under the supervision of a nurse.
  • Complete and document participation in Continuous Quality Improvement activities and enter treatment data into the designated clinical application.
  • Review treatment sheets for completeness, ensure nursing signatures are documented, and correct omitted entries through appropriate staff.
  • Prepare initial patient charts, compile admission paperwork, and prepare laboratory requisitions.
  • Collect information for infection control audits and reinforce infection control practices and education for patients and families as directed.
  • Follow newly admitted patients through their first four weeks of dialysis and coordinate transitional dialysis care.
  • Coordinate with appropriate staff to support the patient experience and ensure scheduled multidisciplinary care.
  • Educate patients about home dialysis products, home dialysis benefits, treatment options, and peritoneal dialysis setup under applicable supervision.
  • Set up and perform treatments on machines used in the transitional care unit.
  • Assist with coordination of the patient’s transition to their selected dialysis modality.
  • Assist with supply ordering and inventory for the transitional care unit.
  • Travel between assigned facilities and various locations within the community and attend regional, business unit, or corporate meetings when required.
