Requirements
  • A Bachelor of Arts or Bachelor of Science degree and a minimum of two years of experience working with a social work agency, or an equivalent combination of education and experience.
  • Specific education, years, and type of experience may be required based on state law and contract requirements.
Responsibilities
  • Conduct face-to-face program assessments using tools with predefined questions and apply motivational interviewing techniques for evaluations, coordination, and management of members’ waiver, behavioral health, physical health, or long-term services and supports needs.
  • Identify members with potential clinical healthcare needs, including potential high-risk complications and gaps in care, and coordinate cases with the clinical healthcare management and interdisciplinary team as the single point of contact.
  • Manage non-clinical needs of members with chronic illnesses, comorbidities, or disabilities to support cost-effective and efficient utilization of long-term services and supports.
  • Document members’ short- and long-term service and support goals in collaboration with the member’s chosen care team, including caregivers, family, natural supports, service providers, and physicians.
  • Identify members who may benefit from an alternative level of service or other waiver programs.
  • Submit utilization or authorization requests to utilization management with documentation supporting and aligning with the individual’s care plan.
  • Report critical incidents to appropriate internal and external parties, including state and county agencies, Adult Protective Services, and law enforcement.
  • Assist with and participate in appeal or fair hearings, member grievances, appeals, and state audits.
  • Facilitate the Person Centered Planning process, including assessments, development of a comprehensive Person Centered Support Plan and backup plan, coordination with Medical Directors, participation in interdisciplinary care rounds, and engagement of the member’s circle of support.
Desired Qualifications
  • A Bachelor of Arts or Bachelor of Science degree in a health care-related field.
  • Bilingual ability in English and Spanish.
  • Experience with long-term services and supports, case management, social work, or hospital discharge planning.
  • Serve as a mentor, subject matter expert, or preceptor for new staff; assist with formal associate training and process improvement initiatives.
