Responsibilities
  • Assess and coordinate medical and supporting services across the continuum of care for complex and high-acuity populations with primary medical or physical health needs.
  • Develop personalized care plans and service plans for long-term care members, address issues, and educate members and their families or caregivers about available services and benefit options.
  • Evaluate service needs for the most complex or high-risk/high-acuity members and recommend plans for the best outcomes.
  • Develop and continuously assess long-term care plans and service plans, collaborating with the care management team to identify providers, specialists, and community resources.
  • Coordinate between members or their families and caregivers and care provider teams to ensure appropriate person-centered care and services.
  • Monitor care plans, member status, changes in condition, and progress toward goals, and revise plans as necessary.
  • Monitor members for complications, clinical symptoms, and status changes, including needs for a higher level of care or waiver eligibility.
  • Review member data to identify trends and improve operating performance and quality of care in accordance with state and federal regulations.
  • Review referral information and intake assessments to develop appropriate care plans and service plans.
  • Collaborate with healthcare providers to facilitate member services or treatments and determine revised care plans when needed.
  • Collect, document, and maintain member information and care-management activities in compliance with state, federal, and clinical guidelines.
  • Provide or facilitate education for members and their families or caregivers on disease processes, care gaps, provider instructions, care options, referrals, and healthcare benefits.
  • Act as liaison and member advocate among members and families, physicians, and facilities or agencies.
  • Educate members about and coordinate community resources, including service authorization for meals, employment, housing, foster care, transportation, and activities of daily living.
  • Perform home or other site visits, such as monthly or more frequently, to assess member needs and collaborate with resources as required.
  • Partner with the leadership team to improve quality of care and service delivery for long-term care members in a cost-effective manner.
  • Precept clinical new hires by fostering core skills, coaching growth, and guiding onboarding when assigned.
  • Provide guidance and support to clinical new hires or preceptees through onboarding, competency checkpoints, and transition from classroom training to field practice when assigned.
  • Engage with People Leaders and cross-functional teams to measure and monitor readiness when assigned.
  • Perform other duties as assigned.
  • Comply with applicable policies and standards.

A modernizer arrives with momentum behind him. Bolivar's résumé reads like a checklist for what a sprawling health insurer needs right now. At Fannie Mae, he pushed enterprise modernization, tightened technology resilience and governance, and expanded the use of AI and automation to improve operations. His background also spans financial services, media and consulting, including a senior leadership stint at Warner Bros. Entertainment, giving him experience outside the narrow confines of healthcare IT. That matters because he is not walking into a company still searching for financial footing. In its July 28 earnings report, Centene posted second-quarter GAAP diluted earnings per share of $2.19 and adjusted diluted earnings per share of $2.51, and it raised its full-year adjusted EPS guidance floor to above $4.80. The company's consolidated health benefits ratio improved to 89.6% from 93.0% a year earlier, and its Commercial segment ratio fell to 79.2%, a notable year-over-year decline. London's comment that Centene is "entering a new era" carries more weight against that backdrop, and Bolivar's own remark about reimagining technology to serve underserved populations lands as more than a talking point when the balance sheet backs it up. Roughly $0.50 of the guidance increase came from non-recurring items in Medicare and Commercial, a detail that tempers how much of the improvement is durable. Bolivar now inherits the job of modernizing technology across three insurance lines moving in different directions, one shrinking, one growing but thinner-margined, and one still working through elevated medical costs, all while a departing CIO's institutional knowledge walks out the door in October. Market sentiment. 72 hedge funds held Centene shares last quarter, unchanged from the quarter before, pointing to steady rather than accelerating institutional interest.
