Requirements
  • An active Michigan Registered Nurse license is required.
  • Reliable transportation, a valid driver's license, and auto insurance are required.
Responsibilities
  • Perform comprehensive in-home assessments and skilled documentation, including OASIS, start of care, recertification, resumption of care, and discharge documentation.
  • Provide direct, high-acuity patient care with a focus on wound care.
  • Build and manage plans of care with physicians.
  • Supervise licensed practical nurses and home health aides on assigned cases.
  • Maintain a full, high-quality caseload and ramp to approximately 30 productivity points per week at target visit quality.
  • Own patient outcomes by driving low avoidable-hospitalization rates and strong patient-experience scores.
  • Participate in case conferences and team huddles to improve quality.
  • Represent Adaptive to patients, families, and referral sources.
  • Set documentation, visit cadence, and clinical judgment standards for future cohorts.
Desired Qualifications
  • Home health experience is strongly preferred.
  • Home health and OASIS experience are strongly preferred.
