Requirements
  • A certificate from an accredited Licensed Practical Nurse program or the required education described by the New York State Education Department is required.
  • A certificate from an accredited school of Practical Nursing is required.
  • A New York State Licensed Practical Nurse license is required.
  • Good clinical patient-care skills are required.

Responsibilities
  • Deliver nursing care to residents and patients in a long-term care facility under the supervision of the Nurse Manager, Nurse Coordinator, and/or Registered Nurse.
  • Recognize changes in residents' or patients' conditions and respond appropriately.
  • Communicate with the Nurse Manager and other healthcare-team members about residents' and patients' needs or changes in condition.
  • Assign duties to Certified Nurse Assistants based on resident and patient needs, staff availability, and unit needs.
