Requirements
  • An unrestricted Illinois physician license is required before the start date.
  • Board certification or board eligibility through the American Board of Family Medicine or American Board of Internal Medicine is required.
  • A clinical doctorate is required.
  • Competence in delivering healthcare services to adolescents, adults, and geriatric patients is required.
  • A physician license in the state of practice is required.
  • Basic Life Support certification is required.
  • A state controlled-substance license and federal Drug Enforcement Administration registration must be maintained when applicable to the specialty.
  • Successful completion of, or current enrollment in, an accredited residency program is required.
  • The candidate must be board certified or board eligible.
  • COVID-19 vaccination or an approved medical or religious exemption is required.
Responsibilities
  • Deliver healthcare services to adolescent, adult, and geriatric patients in a 100% outpatient primary-care practice.
  • Provide patient care Monday through Friday on either a four-day or five-day work week.
  • Use the Epic electronic medical record system with DAX Copilot to assist with documentation.
  • Coordinate specialty-care referrals and prior authorizations with the referral team.
