  • High school diploma or equivalent required.
  • Valid New York, New Jersey, Connecticut, or Pennsylvania Driver’s License with 6 months of experience and 6 or less points.
  • Minimum of 1 year of experience in healthcare coordination, patient advocacy, or related field, with knowledge of developmental disabilities preferred.
  • Strong organizational, communication, and interpersonal skills; ability to prioritize tasks, problem-solve effectively, and navigate healthcare systems on behalf of persons with disabilities.
  • AMAP, SCIP, CPR, and First Aid Certifications preferred.
  • CPR and First Aid certification preferred; additional training in disability services, medical administration, or crisis intervention a plus.
  • Demonstrated strong commitment to safety.
  • Education level: High School / GED.
  • Related: healthcare coordination, patient advocacy, or disability services experience 1+ year.
Responsibilities
  • Schedule and coordinate medical appointments, including primary care visits, specialist consultations, therapy sessions, and diagnostic tests, ensuring adherence to persons needs and preferences.
  • Follow-up when any rescheduled appointments, specifying details, and managing appointment reminders.
  • Provide or arrange transportation for persons to and from medical appointments, ensuring safety and comfort during transit; accompany persons as needed to facilitate communication and understanding between the individual and their provider.
  • Advocate for the healthcare rights and needs of persons with developmental disabilities, ensuring access to appropriate medical care, accommodations, and support services.
  • Maintain accurate records timely of medical appointments including diagnoses, medications, and treatment plans, persons health information, and communication with healthcare providers; report findings participate in clinical meetings, and recommendations to interdisciplinary teams and caregivers.
  • Provide information and education to persons and caregivers on healthcare procedures, medications, and preventive care measures to promote health literacy and self-management skills.
  • Respond to medical emergencies or crisis during appointments, implementing protocols for immediate assistance and coordinating follow-up care as necessary. Ensure follow-up occurs with the team post appointment and any recommendations.
  • Collaborate with healthcare professionals, caregivers, and community resources to facilitate continuity of care, address health concerns, and advocate for persons' well-being.
  • Participate in the Corrective action plans, survey audit follow-up, and collaboration with other staff, nurses, care coordinators, and managers when needed.
Desired Qualifications
  • Healthcare-related certification or degree a plus.
  • CPR and First Aid certification preferred.
  • AMAP, SCIP, CPR, and First Aid Certifications preferred.
  • Knowledge of developmental disabilities preferred.
  • Experience working with developmental disabilities in a healthcare coordination context preferred.
