Requirements
  • Education required to obtain a state nursing license as an LPN/LVN or RN.
  • Current LPN licensure in good standing.
  • Current CPR certification.
  • Strong understanding of the English language sufficient to read, write, and interpret administrative information.
  • Ability to continuously walk and stand, work both indoors and outdoors, and sit infrequently.
  • Ability to lift, carry, push, climb, bend, squat, twist or rotate, and reach according to the stated frequency and weight requirements.
  • Ability to complete required state, compliance, HIPAA, and federal regulatory training and maintain appropriate continuing education units for the license.
Responsibilities
  • Provide nursing support as directed by the Health and Wellness Director.
  • Assist with ensuring resident-care standards and state requirements are met and assist with regulatory compliance.
  • Participate in incoming and outgoing shift reports.
  • Monitor documentation systems and ensure regulatory timeframes for resident assessments and service plans are met.
  • Write and review resident assessments, evaluations, and care or service plans with appropriate staff.
  • Directly supervise nursing-department associates while on shift.
  • Complete the Quality Services report when the Health and Wellness Director is absent.
  • Supervise or participate in narcotic counts at shift change and ensure accurate inventory documentation.
  • Observe medication passes and administer medication as directed in compliance with the Rights of Medication Administration.
  • Participate in weekly medication-cart audits and assist with reordering medication.
  • Maintain medication-room cleanliness.
  • Fax new medication orders to the pharmacy and inform families when mail-order medicines need to be reordered.
  • Communicate with physician offices to obtain phone orders or change orders for residents.
  • Follow Physician Order Protocol for processing orders.
  • Document resident changes in condition or status according to policy.
  • Complete chart audits and thinning according to policy.
  • Assist with required assessments, service plans, and resident care-plan development and maintenance.
  • Document incident reports correctly and promptly.
  • Update do-not-resuscitate orders as needed.
  • Assist the Health and Wellness Director in creating and maintaining resident charts and care files.
  • Visit residents in hospitals or rehabilitation facilities as assigned.
  • Participate in annual flu clinics and tuberculosis testing for associates and residents.
  • Participate in community wellness programs, weekend and holiday MOD coverage, and the on-call schedule as assigned.
  • Serve as a member of the backup sales team.
  • Obtain and record residents' vital signs and weight.
  • Observe residents for medication responses and report unexpected reactions to designated personnel.
  • Counsel residents in identifying and resolving social or other problems.
  • Schedule doctor appointments based on resident needs.
  • Ensure call lights are answered promptly.
  • Attend required training and conferences and maintain current nursing knowledge and license-related continuing education.
  • Ensure caregiver duties are performed and documented appropriately.
  • Assist with oversight of nursing-staff training to meet state regulatory requirements.
  • Address associate, family, and resident concerns.
  • Participate in fire and disaster drills and follow evacuation and disaster policies.
  • Maintain confidentiality of resident and staff personal and health information.
  • Follow compliance and ethics program policies and procedures.
  • Assist with recruiting and interviewing candidates.
  • Perform other duties related to community operations and resident care.
