Phoenix Home Care & Hospice provides in-home health services including skilled nursing, hospice and palliative care, private duty care, and support for developmental disabilities. Care is delivered at patients' homes and financed by Medicare, Medicaid, and private pay, which can reduce hospitalizations and delay move to facility-based living. The company runs a Comprehensive Disease Management program to help patients with complex health needs and an internal PHC U-Too training program to grow and retain staff. Its mission centers on compassionate, Christ-like care, and its goal is to improve patients’ quality of life while managing costs and keeping people out of hospitals.
