Requirements
- A current, unrestricted California registered nurse license is required.
- Three years of experience as a registered nurse is required.
- Proof of a negative tuberculosis result is required.
- Physical screening documentation from within the last year is required.
- An active California driver's license and car insurance are required.
- A cell phone with internet access is required.
Responsibilities
- Complete assessments of clients and their needs.
- Educate clients and families.
- Provide chronic care management.
- Develop care plans and establish goals.
- Provide post-surgical and wound care as needed.
- Provide wellness support.
- Collaborate with interdisciplinary team members to develop and implement individualized care plans.
- Provide ongoing feedback to clients, client representatives, and nurses.
