Requirements
  • Graduated from an accredited nursing program within the past 12 months.
  • Obtain a nursing license and registration within the timeline defined by state regulations or within 90 days of hire, whichever occurs first.
  • Hold a current appropriate state Temporary Permit.
  • Meet the practice requirements in the state of employment.
  • Maintain all required credentials and certifications in active and valid status.
  • Pass the Ishihara Color Blindness test, subject to consideration of reasonable accommodation.
  • Be able to handle heavy lifting and moving patients, assist with ambulation, stand for prolonged periods, bend over, and occasionally move equipment weighing up to 200 pounds with assistance.
  • Be able to work in an environment involving potential exposure to infectious and contagious diseases or materials.
  • Maintain regular and punctual attendance.
Responsibilities
  • Function as part of the hemodialysis healthcare team under the direction or supervision of a Staff Registered Nurse to provide daily quality patient care.
  • Direct Patient Care Technicians in the safe and effective delivery of chronic hemodialysis therapy.
  • Delegate tasks to LVN/LPNs, Patient Care Technicians, and Dialysis Assistants.
  • Assess daily patient-care needs and develop appropriate patient-care assignments.
  • Monitor patient-care staff for proper techniques and adherence to facility policies and procedures.
  • Participate in staff training, new-staff orientation, and required staff meetings.
  • Educate patients and families about End Stage Renal Disease, renal disease, vascular access, dialysis therapy, diet, fluid, medications, laboratory values, treatment compliance, transplant awareness, and treatment modalities.
  • Provide safe and effective care to patients with End Stage Renal Disease and implement prescribed hemodialysis treatment protocols, including sodium and ultrafiltration modeling when appropriate.
  • Assess patient responses to hemodialysis, adjust treatment plans as indicated, and communicate problems or concerns to the Team Leader or physician.
  • Initiate nursing assessments and continuously evaluate and document patient-care needs.
  • Participate in pre-evaluation, initiation, monitoring, termination, access homeostasis, and post-evaluation of hemodialysis treatment.
  • Troubleshoot access-flow issues and intervene when patient adequacy status changes.
  • Provide, supervise when applicable, and monitor hemodialysis access care.
  • Administer and document prescribed interdialytic transfusions and report adverse reactions appropriately.
  • Review, transcribe, and enter physician laboratory orders into the Medical Information System.
  • Prepare laboratory requisitions and ensure correct specimen tubes, collection, processing, packaging, and delivery procedures.
  • Follow up on missed or insufficient laboratory specimens and report abnormal or critical laboratory results to physicians.
  • Maintain safe and efficient shift operations and enforce company, state, and federal policies and regulations.
  • Collaborate with physicians and healthcare-team members to interpret, adjust, and coordinate patient care.
  • Provide assistance with prescription refills according to policy.
  • Oversee patient-information documentation and maintain the facility drug list for required stock medications.
  • Maintain competency in emergency procedures, initiate cardiopulmonary resuscitation and emergency measures when required, and ensure emergency equipment is available.
  • Administer prescribed and as-needed medications, document effectiveness and medical justification, and maintain controlled-substance records.
  • Coordinate patient transportation when necessary.
  • Maintain a clean, safe, and sanitary dialysis treatment environment and operate dialysis-related equipment safely and effectively.
  • Ensure patient stations and equipment are clean and properly positioned and address blood spills according to bloodborne-pathogen-control procedures.
  • Enter physician orders, laboratory results, vital signs, treatment parameters, and patient status into the Medical Information System.
  • Implement patient admission, transfer, and discharge procedures and verify Patient Care Technician documentation.
  • Review daily Hemodialysis Treatment Sheets and ensure opening and closing procedures are completed safely.
  • Initiate and document monthly Continuous Quality Improvement activities and complete monthly nursing progress notes.
  • Maintain complete and accurate patient medical records and documentation.
  • Complete patient care plans, long-term programs, transplant-status reviews, annual nursing histories, physical assessments, and standing-order reviews.
