Requirements
  • Registered Nurse licensed by Massachusetts Board of Registration in Nursing
  • Minimum of 2 years clinical experience, 1 year of experience with elders or adults with disabilities
  • Valid driver’s license with an insured vehicle for travel within the state

Responsibilities
  • Provides support to the caregiver and member, and reviews services according to scope of Adult Foster Care
  • Respects client and employee rights and privacy, ensure the security of protected information, practices in an ethical manner and is compliant to the agency’s compliance program and privacy policy
  • Follows the FreedomCare’s clinical policy and procedures
  • Assesses and regularly reassess the client’s needs and their PCP summary form, treatment and education and communicates any changes to the member’s PCP
  • Develops implements, evaluates, revises, and provides input to the client’s plan of care to include the physician’s plan of treatment, and problem list. Provides instruction of care to Caregiver according to the plan of care
  • Completes all client documentation, to include written records, reports, and notes in accordance with agency policy and procedure following client visits or contacts in an accurate, timely, complete, concise and legible manner
  • Actively coordinates the care of the client through communication and written documentation to the physician, RN and Program Director, other disciplines involved in the client’s care and community resources
  • Interprets the philosophy and objectives of the agency to clients, families, and others
  • Performs other job-related duties as may be required or directed by the supervisor
Desired Qualifications
  • Bilingual in Spanish is a plus
  • Experience with CRM (Salesforce is a plus)
