Requirements
  • Licensed or credentialed health professional (e.g., RN, PA, LCSW, RD, PT, or Master's-level health professional) with experience in diabetes care and education.
  • CCM (Certified Case Manager) certification required.
  • Certification in utilization or care management strongly preferred.
  • CDCES (Certified Diabetes Care and Education Specialist) required, designation and relevant experience preferred.
  • Experience in case management, care coordination, managed care, or utilization management.
  • Strong organizational and prioritization skills with the ability to manage a caseload of complex cases effectively
  • Excellent communication skills – verbal, written, and interpersonal.
  • Proficiency in MS Office (Word, Excel, Outlook, PowerPoint) and EMR systems.
  • Experience in clinical settings and with electronic medical records.
  • Bilingual ability (verbal and written) is preferred.
  • Strong critical thinking, problem-solving, and collaborative skills.
  • Ability to work a flexible schedule, including evenings, weekends, and holidays as needed.
Responsibilities
  • Conduct comprehensive assessments to identify members' medical, psychosocial, financial, caregiving, environmental, and benefit-related needs.
  • Develop and implement individualized care plans that include clearly defined goals and appropriate interventions, including referrals to internal and external resources.
  • Collaborate with primary care providers (PCPs), caregivers, community-based organizations, and multidisciplinary teams to support and empower members.
  • Facilitate communication across stakeholders through telephonic and face-to-face case conferences as needed.
  • Act as an advocate and liaison for members, coordinating services with healthcare providers and private, non-profit, and governmental agencies.
  • Monitor and evaluate care plan effectiveness, adjusting strategies as needed to ensure quality, cost-effective outcomes.
  • Assign members to the appropriate care team resources (e.g., registered dietitians, social workers, community health workers) based on assessed needs.
  • Ensure compliance with federal/state regulations, NCQA standards, and internal protocols.
  • Maintain accurate and timely documentation in the electronic medical record (EMR) system.
  • Stay up-to-date on evidence-based guidelines and medical protocols to inform care management decisions.
  • Actively participate in departmental committees, training, and quality improvement initiatives.
  • Experience with diabetic devices (Libre, Dexcom, etc)
  • Candidate works closely with the endocrinologist
Desired Qualifications
  • - Certification in utilization or care management strongly preferred.
  • -CDCES (Certified Diabetes Care and Education Specialist) preferred, designation and relevant experience preferred.
  • -Bilingual ability (verbal and written) is preferred.
  • -Experience in diabetes devices other than listed (if any) preferred.
  • -Prior experience in a similar role in a healthcare setting preferred.
