Requirements
  • High School graduate or GED
  • Completed training program in Medical Assisting from Accredited College
  • Possess a valid Lifesaving certificate
  • Minimum 2 years of experience in a health care setting, preferably in a Federally Qualified Health Center
  • Previous experience working in Behavioral Health Department
  • Bilingual (English/Spanish) required
  • Able to quickly build and maintain rapport with patients and providers of differing backgrounds; team player

Responsibilities
  • Escorts patients to the tele-psychiatry exam room, administers Outcome Measures (Audit-C, PHQ-9, GAD-7, DAST, and CAGE), measures and records vitals, in order to facilitate visit with psychiatric team
  • Performs administrative responsibilities, verifying demographics, assisting patients with filling out necessary paperwork, and making up follow-up appointments
  • Identifies any Behavioral Health gaps in care, and any quality metrics that need to be met by reviewing patient’s historical electronic medical record
  • Participates in regularly scheduled caseload consultation with team care including Primary Care, Behavioral Health Provider and/or Psychiatrist
  • Facilitates patient engagement and follow up in care with Behavioral Health and/or Tele-psychiatry
  • Facilitates referrals for clinically indicated services internally or outside the primary care clinic (e.g. Social services, mental health specialty care, and substance abuse treatment)
  • Is responsible for appointment follow through and follow up, will track and document no shows for appointments
  • Assists with any additional documentation or requirements as they relate to patient’s prescriptions (i.e., TARS, Prior Authorizations), Assists patients with any medication concerns/requests
  • Maintain patient confidentiality in accordance to department policy and HIPPA guidelines
  • Assists with and Triages Crisis Calls if no Behavioral Health Provider is available
  • Attend and participate in required meetings, training and educational programs
  • Perform other functions as directed by the Director of Behavioral Health
