Requirements
  • Two years of Nurse Practitioner experience in an acute care environment.
  • Home Health, Palliative Care, or Hospice experience is required.
  • A Master of Science in Nursing is required.
  • An active, unencumbered Ohio Nurse Practitioner license is required.
  • A current driver's license and auto liability coverage are required.
  • Recertification for Hospice patients is required as part of the detailed assessment responsibilities.
  • A Drug Enforcement Administration number is required.
  • A National Provider Identifier is required.
