Requirements
  • For Case Manager I: Bachelor of Arts degree in Social Work, Psychology, or a related human services field and one year of similar experience; or a Bachelor of Arts in an unrelated human services field plus four years of experience working directly with individuals with developmental disabilities.
  • For Case Manager II: Master’s Degree in Social Work, Psychology, or a related human services field.
  • Ability to speak Cantonese and Mandarin fluently.
  • Possession of a valid driver's license and access to an automobile.
  • Ability to communicate clearly and effectively in both verbal and written forms.
  • Demonstrated cultural awareness and sensitivity with the ability to work effectively with culturally diverse populations.
Responsibilities
  • Facilitate the planning process and develop the Individual Program Plan or Individual Family Service Plan for assigned consumers.
  • Oversee, monitor, and evaluate the implementation of services provided through the Individual Program Plan or Individual Family Service Plan.
  • Coordinate and support the implementation of consumer plans by identifying natural supports, generic services, and evaluating needs that require Regional Center of the East Bay authorized funding.
  • Advocate on behalf of consumers to protect their legal rights and ensure they receive entitled services and benefits.
  • Assist consumers and families in obtaining essential diagnoses and evaluations from practitioners and secure written reports assessing the consumer’s situation.
  • Monitor the health and safety of consumers.
  • Maintain all documentation and contacts as required by law, regulation, and policy.
  • Implement Regional Center of the East Bay policies and procedures.
  • Complete functional and developmental reviews in a timely manner.
  • Inform supervisors regarding caseload status and unmet community needs.
