  • Proficiency in tech platforms: Google Suite, EHR systems
  • Comfortable providing telephonic, written, digital, and face-to-face outreach, and running group outreach/events
  • Strong customer service mindset and a passion for Cityblock's mission and members
Responsibilities
  • Locate and meet with eligible members in their homes, neighborhoods, and the Cityblock health hub
  • Reviewing pre-populated demographic information and documenting accurate information and emergency contacts within Commons
  • Leveraging available data from Commons to learn more about potential members and incorporate adjusted strategy into your outreach process based on that data/information
  • Completing the consent process for eligible members fully and accurately, including obtaining written consent per member
  • Engage with and serve as the initial contact and representative of Cityblock
  • Effectively communicate urgent needs and other pertinent information in an accurate and timely manner to care teams
  • Working daily within Commons to collect data, organize information, track tasks, monitor performance, and communicate with staff, patients, and family members
  • Providing feedback to the product development team and becoming part of a super-user group to assist in onboarding and supporting others
