Requirements
  • A Registered Nurse with a master's degree specializing in psychiatric or mental health nursing must be licensed in the state where practicing and have nursing experience, recommended within the last three years, in an acute treatment unit in a psychiatric hospital, psychiatric home care, psychiatric partial hospitalization program, or other outpatient psychiatric services.
  • A Registered Nurse with a bachelor's degree in nursing must be licensed in the state where practicing and have one year of recent nursing experience, recommended within the last three years, in an acute treatment unit in a psychiatric hospital, psychiatric home care, psychiatric partial hospitalization program, or other outpatient psychiatric services.
  • A Registered Nurse with a diploma or associate degree in nursing must be licensed in the state where practicing and have two years of recent nursing experience, recommended within the last three years, in an acute treatment unit in a psychiatric hospital, psychiatric home care, psychiatric partial hospitalization program, or other outpatient psychiatric services.
  • Current cardiopulmonary resuscitation certification is required.

Responsibilities
  • Make the initial assessment visit using observation and assessment skills and assist in developing the patient's plan of care in coordination with the patient's physician or authorized practitioner and Clinical Manager.
  • Manage medical illness according to the plan of care and perform psychobiological interventions.
  • Conduct comprehensive medication reviews and ensure patients and caregivers receive information about medication frequency, duration, use, side effects, indications, and contraindications.
  • Educate patients and families about medication administration, actions, and side effects.
  • Provide written materials for patients and families.
  • Evaluate, teach, and review medications and compliance, and administer intramuscular or intravenous medication.
  • Manage situational and other crises, assess potential self-harm or harm to others, and refer patients to treating physicians when necessary.
  • Provide skilled nursing teaching and training to patients, families, or caregivers on treatment regimens, self-care, mental and physical well-being, independence, and patient rights.
  • Promote and encourage patients and caregivers to maintain a safe and therapeutic environment.
  • Provide supportive counseling, psychotherapy, and psychotherapeutic interventions within the scope of education and licensure.
  • Provide psychoeducation on disease processes, symptoms, safety management, coping skills, and problem solving.
  • Continuously evaluate and manage the patient's care plan.
  • Provide counseling services or coordinate with a social worker to ensure counseling services are provided.
  • Document evaluation, psychotherapy, and teaching activities in accordance with coverage guidelines for patients with diagnosed psychiatric disorders requiring active treatment by a psychiatrically trained nurse.
  • Provide general nursing interventions, including phlebotomy, wound care, infusion, ostomy care, catheter care, and other skilled interventions, as ordered by the physician.
  • Provide case management to enhance patient outcomes, including communication and coordination with other disciplines.
  • Monitor patient conditions and report changes to the Clinical Manager and physician or authorized practitioner when indicated.
  • Obtain, document, and execute orders from the patient's physician or authorized practitioner.
  • Ensure compliance with state, local, and payer requirements and complete documentation within required timeframes.
  • Participate in after-hours coverage, including weekends and holidays, when necessary and requested.
  • Supervise care provided by licensed practical nurses and home health aides when indicated.
  • Maintain universal precautions and infection-control standards.
  • Disinfect, sanitize, and transport patient-care equipment and supplies and use proper medical-waste disposal techniques.
  • Report incidents according to agency policy and regulatory standards.
  • Provide professional guidance to referral sources and internal operational staff.
  • Assist with staff development and provide one-on-one clinical training to field registered nurses and licensed practical nurses during home visits.
  • Collaborate with the Clinical Manager, Branch Director, or Administrator to maintain staff competencies.
  • Meet with and educate referral sources about agency services by providing in-services when requested.
  • Sustain and grow established accounts and identify and develop new referral sources through prospecting, lead generation, and sales calls within an assigned territory when requested.
  • Attend and participate in staff meetings, patient-care conferences, in-services, and other meetings as requested.
  • Participate in the agency's Quality Assurance and Performance Improvement program by communicating problem areas and recommendations, attending periodic meetings, and participating in performance-improvement projects when requested.
  • Maintain confidentiality of patient-related information and honor patient rights.
  • Perform other duties as assigned and within the scope of practice.
