Requirements
  • A high school diploma/GED and/or Peer Certification is required.
  • Proficiency in Microsoft Office and database software is preferred.
  • Ability to function as part of an interdisciplinary team is preferred.
  • Ability to maintain patient confidentiality at all times is required.
Responsibilities
  • Facilitate monthly and/or quarterly telehealth video calls for team providers.
  • Assist with tele-visits and train clients to use telehealth.
  • Create individualized wellness action plans in collaboration with providers and community-based organizations, addressing health, transition to outpatient programs, and housing connections.
  • Provide ongoing client follow-up regarding graduation and housing goals.
  • Conduct telehealth and field visits as needed, with no more than 10–12 field visits per month.
  • Support two-person visits for high-risk clients.
  • Escort clients to Assisted Outpatient Treatment appointments.
  • Identify community medical and mental health resources and provide information and referrals to clients.
  • Assist clients with medication reviews and understanding provider directives to address mental health conditions and reduce hospital utilization.
  • Coordinate transportation resources, escort clients to medical appointments, and provide appointment reminders.
  • Support recreational activities.
  • Coordinate scheduling of meetings with consumers alongside other team members.
  • Participate in group planning, implementation, and processing as requested.
  • Fulfill scheduling requirements.
Desired Qualifications
  • A bachelor's degree in a related field, mental health, or social work is preferred.
  • Experience in the mental health service field is preferred.
  • Compassion, high-quality care, and dedication are preferred.
  • Strong collaboration, organizational, and time-management skills are preferred.
  • A person-centered, values-based approach to services is preferred.
  • Keen attention to detail is preferred.
  • Effective written and oral communication skills, including the ability to articulate and communicate messages for internal and external audiences, are preferred.
  • Communicating, planning, and coordinating with team members is preferred.
  • Representing the organization positively and professionally in the community is preferred.

S:US provides services and advocacy for New Yorkers facing homelessness, intellectual/developmental disabilities, behavioral health and addiction challenges, poverty, criminal justice involvement, and trauma. It supports tens of thousands of individuals and families in New York City and Long Island with about 1,800 staff. The organization offers tailored direct services such as housing assistance, health and behavioral supports, case management, and community programs to help clients achieve stability and long-term well-being. Its goal is to empower underserved New Yorkers to lead lives of purpose, improve neighborhoods, and break the cycle of poverty for current and future generations.
