Requirements
  • Minimum high school education and two years of experience in human services, addiction treatment, or a related field.
  • Basic computer skills, including competence with Microsoft Office Suite, Adobe, internet applications, wireless computer hardware, and projecting to remote devices.
  • Ability to work independently and collaboratively with Clinical Treatment, Admissions, and Business Development teams.
  • Ability to multitask and prioritize specific caseload needs.
  • Ability to adapt to a changing environment.
  • Strong communication skills.
  • Excellent organization and time management skills.
Responsibilities
  • Develop and provide clients with continuing care plans and resources before discharge.
  • Market and promote the treatment center's programs and facilities by preparing and providing information and answering questions.
  • Accurately relay payment information necessary for admission, collect insurance information, and communicate financial options.
  • Collaborate with the client's treatment team and family wellness staff to develop clinically appropriate aftercare plans.
  • Complete all daily scheduled tasks.
  • Manage a caseload of up to 15 clients.
  • Provide family members, providers, and referral sources with weekly updates on continuing care plans and clients' commitment to aftercare planning.
  • Facilitate weekly group sessions educating new residential clients about the continuing care process and appropriate levels of care and providers.
  • Maintain notes and applicable documentation in customer relationship management and electronic medical record systems.
  • Comply with federal, state, and accreditation regulatory requirements.
  • Perform other assigned duties.
Desired Qualifications
  • A Bachelor of Science, Bachelor of Arts, or Bachelor of Social Work degree from a four-year accredited college or university.
