- A high school diploma or equivalent is required.
- A valid New York, New Jersey, Connecticut, or Pennsylvania driver’s license, at least six months of driving experience, and six or fewer license points are required.
- The role requires interpersonal and communication abilities, patience, empathy, and a genuine desire to support people with developmental disabilities.
- The role requires the ability to respond effectively to changing needs and environments, demonstrate flexibility and problem-solving skills, maintain confidentiality, uphold ethical standards, and follow agency policies and procedures.
- The role requires a strong commitment to safety.
- Implement and monitor varying plans of care for the people supported, including comprehensive functional assessments, staff action plans, life plans, treatment plans, and behavior support plans.
- Implement and document goals and objectives from person-centered plans through structured activities and positive reinforcement.
- Provide direct care and assistance with daily living activities, including personal hygiene, meal preparation, medication administration, and transportation.
- Facilitate community outings, recreational activities, and social events, and accompany people supported to appointments and healthcare follow-ups.
- Use person-centered approaches and positive behavior support strategies to address challenging behaviors and provide the required supervision and support for medical, developmental, clinical, and emotional needs.
- Maintain accurate records, daily progress notes, and incident reports concerning achievements, behaviors, and health status in compliance with agency policies and regulatory requirements.
- Collaborate with healthcare professionals, therapists, and family members to develop and implement comprehensive care plans.
- Advocate for the rights, choices, and preferences of people with developmental disabilities and promote self-advocacy and empowerment in decision-making.
- Serve as a custodian and mandated reporter in accordance with OPWDD, NYS Justice Center, and organizational policies.
- Administer medications according to prescribed schedules, accurately document medication administration, and monitor for adverse reactions or side effects.
- Experience in direct care or human services is preferred.
- CPR and First Aid certification are preferred.
- Additional training in developmental disabilities, behavior management, or healthcare is desirable.
