Responsibilities
  • Performs comprehensive nursing assessment and reassessment including age-appropriate admission assessment or transfer assessment and input from family/guardian; documents findings accurately and completely; assesses post-operative/post-invasive procedure patients; evaluates education and discharge needs on admission and throughout hospitalization.
  • Establishes, coordinates and evaluates a plan of care based on analysis of assessment data, patient diagnosis, lab data, tests, procedures, physician orders, protocols and standards of care; identifies short and long term goals and formulates nursing interventions to achieve desired patient outcomes; incorporates disease-specific evidenced-based practice into nursing care plan and documentation.
  • Provides nursing interventions based on assessed patient needs, plan of care, and changes in patient status; collaborates with health team members for coordination of daily plan of care; provides accurate handoff communication; administers and documents medications according to policies and procedures; monitors IV fluids and parenteral nutrition; completes referrals; requests consultations for special needs; provides patient/family education and discharge planning; clarifies physician orders as warranted; acknowledges and implements physician orders in a timely manner and assists physicians with procedures or delegates as appropriate; documents readback for telephone/verbal orders and only takes such orders in emergencies; recognizes changes in patient condition and takes appropriate nursing actions; involves family/guardian in care and decision-making; adheres to infection control practices.
  • Documents and/or communicates nursing care and/or changes in patient condition; performs ongoing evaluation of effectiveness of care; evaluates patient/family education; evaluates and modifies plan of care as indicated; reports significant changes to physician and other staff; identifies problems and pursues solutions; reports variations from care per occurrence policy.
  • Provides specialized care to high-risk patients including restraint care with alternatives considered prior to application, monitoring and documenting restraint use; pain management including assessing pain, incorporating cultural/spiritual beliefs, implementing prevention-focused plan, involving patient/family; abuse assessment including recognition criteria, reporting suspected abuse/neglect and ensuring safety.
  • Demonstrates accountability for own professional practice by adhering to quality standards, maintaining currency in hospital/unit information and policies, attending meetings, participating in committees, and updating knowledge; adapts to changing work demands and supports peers; relates professionally with coworkers and acts as a resource and supervisor as needed.
  • Performs safety and infection control duties including following standard precautions, maintaining safe and clean environment, following hand hygiene and Safe Patient Handling practices, reporting risk management concerns, completing annual mandatory requirements, and complying with policies.
  • Delivers service excellence by providing prompt, respectful, courteous care; communicates effectively and maintains confidentiality; demonstrates competence and collaboration; uses resources wisely to improve efficiency and minimize waste; ensures patient satisfaction through adherence to hospital principles and commitments; incorporates performance improvement and evidence-based practice into practice; participates in unit-based and hospital performance improvement activities and nursing research when applicable.
  • Provides age-appropriate care by applying knowledge of growth and development to patient needs, interpreting age-specific data, involving family in decision-making, and serving the age groups routinely served by the department.
  • Supports cost-effective services by using resources efficiently, documenting chargeable supplies, prioritizing tasks, and using non-clinical time for required education; performs other related duties as assigned.
