Requirements
  • At least one year of recent experience as a professional nurse in home health is required.
  • A valid driver's license with no more than three moving violations is required.
  • A Registered Nurse credential is required.
  • Basic Life Support certification from the American Heart Association is required.
  • The candidate must be a graduate of an accredited school of nursing.
Responsibilities
  • Assess and admit eligible patients for home health care services.
  • Conduct comprehensive OASIS assessments in accordance with regulatory and organizational standards.
  • Evaluate patient needs and determine appropriate levels of home health services.
  • Develop individualized plans of care based on patient assessments, physician orders, and clinical standards.
  • Implement, monitor, and revise care plans to ensure high-quality, safe, and effective patient outcomes.
  • Provide skilled nursing services and comprehensive clinical assessments in the home setting.
  • Perform and manage physician-ordered PleurX drain care, finger-stick coagulation checks, wound care and wound VAC management, intravenous therapy, phlebotomy, tracheostomy care, PEG tube care, ostomy care, and telehealth monitoring and support.
  • Educate clients and caregivers on disease management, medication administration, treatments, and safety.
  • Provide supervision and clinical guidance to patients and family members to promote independence and self-care.
  • Offer counseling to support patient understanding and participation in the care plan.
  • Collaborate with the multidisciplinary home health team to ensure coordinated, patient-centered care.
  • Communicate with physicians, therapists, social workers, and supervisors regarding patient status and care needs.
  • Complete accurate and timely OASIS documentation in compliance with regulatory and agency standards.
  • Maintain thorough and clear clinical records to support quality care and continuity of services.
  • Use critical thinking and clinical judgment to identify patient needs and respond effectively.
  • Address patient and caregiver concerns promptly using problem-solving strategies.
  • Participate in required weekend and holiday rotations.
  • Perform other duties as assigned to support patient care and agency operations.
