Requirements
  • Doctor of Medicine or Doctor of Osteopathy degree from an accredited institution.
  • Completion of residency training in a relevant specialty, including Internal Medicine, Family Medicine, Neurology, Oncology, or a related field.
  • Board certification or eligibility in Palliative Medicine or a primary specialty.
  • An active medical license to practice in the state of employment.
  • Experience managing complex chronic illness.
  • State certification and/or licensure must be maintained.
Responsibilities
  • Evaluate and manage patients with serious or chronic illnesses in an outpatient palliative care setting.
  • Provide symptom management for pain, dyspnea, nausea, fatigue, anxiety, and depression.
  • Conduct goals-of-care conversations and advance care planning, including discussions about prognosis and treatment preferences.
  • Collaborate with primary care providers, specialists, social workers, nurses, and other interdisciplinary team members.
  • Support patients and families with complex medical decision-making and care coordination.
  • Refer and coordinate with hospice services and community resources as appropriate.
  • Maintain accurate, timely, and compliant medical documentation.
  • Participate in quality improvement initiatives, program development, and continuing medical education.
