Requirements
  • Two or more years of experience working with social service delivery to an elderly population.
  • Working knowledge of resources available in the geographic location serving the community.
  • Proficiency operating a computer, the Internet, Microsoft Office, other relevant software, and basic office equipment.
  • Proven experience in service management or facilitation, including organizing, problem-solving, and advocating.
  • Ability to read, write, understand, and speak the English language.
  • Strong interpersonal and communication skills.
  • Ability to work closely with residents, family members, legal representatives, and housing community staff.
  • Ability to function independently with a high level of flexibility and personal integrity.
  • Ability to adhere to all terms and conditions set forth in the United Church Homes Employee Handbook.
Responsibilities
  • Assist and educate clients or residents and families in acquiring and using community services necessary to maintain a self-reliant lifestyle.
  • Act as a liaison between community agencies, service providers, and clients or residents; seek new services and identify low-cost providers or negotiate discounts.
  • Monitor service delivery to clients or residents to ensure services are appropriate, timely, and satisfactory.
  • Maintain a resource directory of available community services.
  • Maintain professional relationships with clients or residents in the housing community to identify needs and make referrals to community agencies.
  • Maintain resident files, including intake, assessment, service referral and termination information, quarterly reviews, follow-up, human or civil rights violations, and resident, family, and provider meeting or communication notes.
  • Conduct quarterly quality service reviews and follow-up.
  • Submit required reporting data within specified timeframes.
  • Network with service providers to maintain necessary relationships and pursue additional services through private, local, state, and federal sources.
  • Travel for conferences, trainings, and other company events as required.
  • Promote wellness activities for residents, including arranging onsite or mobile health care services and screenings.
  • Coordinate educational events for residents, families, and staff on community resources, referral processes, and eligibility requirements.
  • Assist residents in building informal support networks with other residents, family members, and friends.
  • Coordinate volunteer support programs with community service organizations.
  • Assess client needs to establish individual service plans.
  • Act as a family support system and provide community resources for caregivers.
  • Deliver home visits during inclement weather when required.
  • Travel daily to client homes and community service providers, as well as to trainings, conferences, and other company events.
Desired Qualifications
  • A Bachelor's Degree in social work is preferred.
  • Membership in and familiarization with the American Association of Service Coordinators is helpful.
