Requirements
  • Professional Licensed as a Clinical Social Worker by the California Behavioral Science Board.
  • Minimum 2-4 years’ experience working with the elderly or disabled adults.
  • Master’s in Social Work required.
  • Must maintain current BLS (If not current, must obtain current BLS within first six months of association with AltaMed Health Services Corporation).
  • BLS certification required.
  • Direct experience with evidence based treatments for individual, group & families therapy sessions (Coping with death/dying, Coping with Stress, Cancer support discussion, etc.).
Responsibilities
  • Applies psychological/counseling principles in assessment, clinical interventions, research, training, case management, consultation, program development and supervises non-licensed staff (when applicable).
  • Diagnosis, evaluation, treatment, discharge planning and supervise mental health services provided to participants.
  • Provides a variety of interventions such as individual, group, couple and family therapy in the PACE setting.
  • Involves integrating behavioral health services with the PACE interdisciplinary team and community agencies to ensure appropriate participant care as outlined in the care plan.
