Requirements
  • A high school diploma or equivalent is required.
  • At least 1 year of administrative, scheduling, or office experience is required; healthcare or hospice experience is preferred.
  • Strong organizational and multitasking skills are required.
  • Experience with electronic medical records or scheduling systems is preferred.
  • Excellent communication and interpersonal skills are required.
  • The ability to prioritize, adapt, and solve problems in real time is required.
Responsibilities
  • Schedule and coordinate patient visits across hospice clinicians to ensure timely, efficient care.
  • Collaborate with nurses, social workers, chaplains, and leadership to support daily care coordination.
  • Monitor schedules and follow up on missed, incomplete, or delayed visits.
  • Use electronic medical records and reporting tools to track productivity, coverage, and scheduling optimization.
  • Partner with field teams and leadership to reduce missed visits and improve the patient experience.
  • Communicate with patients, families, clinicians, and referral partners.
  • Respond to changing patient needs and scheduling challenges with urgency and problem-solving.
  • Maintain accurate documentation and complete workflows within the electronic medical record system.
  • Support quality initiatives, including participation in Quality Assurance and Performance Improvement programs.
  • Promote compassion and professionalism in every interaction.
