Requirements
  • Bachelor’s degree required. MSN preferred
  • Current/valid Florida nursing license from a nationally accredited school of nursing
  • CPR/BLS
  • 3 years of recent nursing experience required
  • Strong interpersonal communication skills
  • Proven track record of building relationships with physicians, direct reports, and key staff
  • Moderate to advanced computer literacy
Responsibilities
  • Demonstrates the use of the nursing process as a problem-solving model
  • Reviews all orders on new admissions, transfers, and discharges
  • Communicates patient changes and condition with the assigned team members as indicated
  • Communicates with patient/family within 24 to 72 hours of admission
  • Attends daily interdisciplinary team rounds
  • Collaborates with Primary RN, case managers/social services during the patient's stay
  • Assumes a proactive role in coordinating interdisciplinary discharge planning for a specific patient population
  • Facilitates access to outpatient resources that the patient may need, in collaboration with the multidisciplinary team
  • Works with APRNs and other Providers throughout the stay to ensure the patient is ready for discharge by noon
  • Demonstrates commitment to meeting the learning needs of patients and families
  • Utilizes appropriate resources to assist in meeting those needs and achieve positive patient outcomes
  • Assists the patient/family in maximizing their sense of control and actively participating in their recovery
  • Supports the patient discharge process in terms of medication reconciliation and finalizing discharge orders
  • Completes the discharge summary, provides patient/family education, and documents discharge teaching
  • Validates patient/family education and comprehension before discharge
  • Facilitates all follow-up appointments and ensures that ordered tests and procedures are scheduled before patients’ discharge
  • Communicates with patient/family 24 hours post-discharge
  • Conducts patient follow-up at the following intervals and as needed (3, 7, 14, and up to 30 days as needed)
  • Serves as a liaison between the medical and surgical teams and patients and their families post-discharge
  • Demonstrates the knowledge and skills necessary to provide care appropriate to the age and needs of the patients served on the assigned unit
  • Serves as a direct care provider, as appropriate, to ensure positive outcomes
  • Maintains a safe and clean environment that promotes the safety of patients and their families throughout all care environments
  • Complies with regulatory standards, including National Patient Safety Goals
  • Demonstrates creativity and innovation in approach to complex problem solving
