Requirements
  • A Bachelor's degree, specifically a B.A., or equivalent experience, is required; alternatively, four years of housing-related experience may qualify.
  • At least one year of previous case management experience is required.
  • Comfort with technology, including basic knowledge of Microsoft Office Suite, Google Suite, and city housing databases, is required.
  • Knowledge of and familiarity with the ONE System is required.
  • Effective verbal and written communication skills are required.
  • Ability to work both independently and cooperatively is required.
  • Ability to de-escalate conflict is required.
  • Extensive understanding of homelessness, trauma, domestic violence and intimate partner violence, HIV/AIDS, substance use disorder and recovery, mental health disorders and neurodiversity, and systems of oppression is required.
  • Ability to engage with others in a positive, energetic, helpful, and trauma-informed manner is required.
  • Ability to work effectively with diverse individuals who self-identify by culture, language, neighborhood, sexual identity, gender, and other definitions is required.
  • Ability to write and speak English effectively is required.
  • CPR and first aid certification are required within 90 days of hire.
  • Ability to stand and walk as needed throughout the shift for extended periods is required.
  • Ability to lift up to 50 pounds is required.
  • Ability to sit and use a computer, including a keyboard and visual display terminal, for extended periods is required.
  • Finger, hand, and wrist agility is required.
  • Ability to complete tasks in an environment with frequent interruptions and background noise is required.
  • Ability to move through buildings, including stairs, between buildings, and in the community, is required.
Responsibilities
  • Establish and maintain a caseload of participants, providing advocacy, connections to community resources, benefits support, and crisis intervention.
  • Meet with participants regularly to assist them in developing and executing an Individualized Service Plan with timelines for individual goals and objectives.
  • Ensure access to resources that stabilize participants' income through benefits advocacy, housing, medical status, social service systems, other agencies, and related needs.
  • Conduct outreach to encourage program usage, especially housing placement through the program's Housing Specialists.
  • Identify participant behaviors that may jeopardize housing and intervene when necessary to promote housing retention.
  • Advocate and link participants to eviction-prevention resources to support housing retention.
  • Maintain participant files and associated documentation.
  • Assist participants in developing external support systems.
  • Adhere to established policies and procedures.
  • Ensure participants are treated with dignity and respect and that their needs are met while participating in the program.
  • Support stabilization, hope, and healing for adults experiencing homelessness in San Francisco.
  • Attend training and meetings.
  • Perform other duties as assigned.

Community Forward SF provides housing, health, and support services to people experiencing homelessness in San Francisco, with a focus on women, gender-diverse individuals, older adults, and those with serious medical or mental health needs. Services are delivered through multiple sites and programs funded by public partnerships, private donors, and foundations, combining shelter, transitional and permanent housing, medical respite, a Sobering Center, a Managed Alcohol Program, housing subsidies, and coordinated case management. It differentiates itself by its targeted client mix and integrated continuum of care, including two permanent supportive housing sites and a leadership model with co-CEOs. Its goal is to empower vulnerable individuals to break cycles of housing instability and violence, reduce homelessness, and improve health and safety, serving more than 6,000 people each year.
