Requirements
  • Masters’ degree in social work
  • License/certification eligible or Licensed (LMSW) preferred in Texas/Florida
  • Minimum 3-5 years of post-graduate experience in health care social work/Case management in behavioral health
  • Experience working with pediatric population and family systems
  • Proficiency in motivational interviewing practices and/or techniques
  • Knowledge of social work including crisis prevention and intervention
  • Experience with providing telehealth services
  • Bilingual Spanish Required
  • Strong knowledge of behavioral health principles and practices
  • Proficient in trauma-informed care practices
  • Strong knowledge of mental health common signs and symptoms
  • Compassion and exceptional communication skills
  • Organizational skills and cultural competence
  • Solution-focused mindset and problem-solving skills
  • Ability to communicate effectively with a diverse audience
Responsibilities
  • Monitor high-risk pediatric patients ensuring appropriate follow-up and clinical management
  • Evaluate effectiveness of care plan and identify need for transitions of care
  • Develop intervention plans for patients and caregivers
  • Provide case management and support for mental health diagnosis
  • Support patient and family system in accessing quality care
  • Provide crisis interventions and training/education on mental health
  • Participate in consultations with interdisciplinary team
  • Represent the organization to patients, families, and community
  • Provide telehealth visits and travel for training
  • Work afternoons and evenings
  • Perform other assigned duties
