Requirements
  • Must be at least 18 years of age.
  • Bachelor’s degree with major course work in human services plus one year full-time work experience in a related field; or an Associate’s degree with major course work in human services plus three years of full-time work experience in a related field, or a High School diploma or GED plus five years of full-time work experience in a related field.
  • Well-developed organizational skills.
  • Ability to work effectively with inspectors, monitoring and licensing agencies.
  • Must have the required annual Tuberculosis testing and annual Health Assessment completed.
  • Must have the required references and criminal record check.
  • Must have a valid New York State driver’s license that meets agency’s clean driving record policy.
Responsibilities
  • Respects and maintains resident rights and confidentiality and demonstrates a caring, positive attitude toward all residents, staff and guests and service providers.
  • Assumes primary responsibility for coordinating and performing services necessary to provide socialization skills.
  • Conduct interviews and evaluate prospective residents to determine appropriateness; tour families and applicants and oversee the complete admission process for all new admissions.
  • Works with residents to encourage healthy connections with family and community as well as develop new interests and form new support systems.
  • Maintains accurate and timely records of case management interventions, appointments, recommendations and follow-up in each resident’s file. This includes developing of a Person-Centered Service Plan upon admission and reviewed/updated every 6 months and as needed in conjunction with the resident’s responsible party, resident’s physician and home care agencies.
  • Provides one-to-one support for residents related to emotional, behavioral, social and emotional concerns.
  • Works with the Resident Care Director to ensure that the residents obtain/maintain a primary physician of their choice and have access to other services as needed to maintain their optimal health/mental health.
  • Actively networks and maintains links with other health care and human service organizations. Make appropriate referrals according to residents’ needs.
  • Assist residents in need of alternate living arrangements to make and execute sound transfer and/or discharge plans.
  • Assists with annual rent increases, coordinating paperwork along with the administrator. Coordinates paperwork when there is a room/rent change.

Pan-American Square Apartments Wins Community Impact Award

Pan-American Square Apartments wins Community Impact Award. SWBR Company is pleased to announce that DePaul Pan-American Square Apartments has been recognized with the 2025 Community Impact Award by the Buffalo/WNY Chapter of the American Institute of Architects. This award honors projects that make a meaningful difference in their communities through thoughtful, inclusive design. Completed earlier this year, Pan-American Square Apartments transformed a former rail corridor into a 150-unit affordable and supportive housing community in Buffalo, NY. Designed to Passive House standards, the development provides energy-efficient homes and essential services for seniors and individuals experiencing homelessness or mental illness. More than housing, it is a catalyst for community revitalization, delivering stability, sustainability, and hope for Buffalo's most vulnerable residents. Jury Comment: The integration of community-based programs and sustainable design at the Pan-American Square Apartments struck the jury. Exemplifying the criteria of the Community Award, the Pan-American Square Apartments took a holistic approach to senior living and supportive housing that will have a lasting positive impact on the WNY community. It is clear that this project will uplift and provide stability for the most vulnerable members of its community and the design team took every opportunity to maximize resources available to its residents. SWBR provided architectural, interior design, structural engineering, landscape architecture, and graphic design services for the project. The awards celebration was held Thursday, November 6 in Buffalo, NY.
