- Graduate of an accredited school of nursing or education equivalency for licensing.
- No experience is required.
- Proof of motor vehicle liability insurance meeting minimum state requirements upon hire and upon request.
- Basic Life Support HealthCare Provider certification from the American Heart Association.
- A registered professional nurse license valid for the applicable state of work location, either through a compact state or the applicable state licensing authority.
- A satisfactory Motor Vehicle Report.
- A state-issued driver license.
- Ability to meet the listed physical requirements, including frequent driving, standing, walking, keyboard use, and occasional patient lifting or moving.
- Assess and coordinate patients’ discharge planning needs with members of the healthcare team.
- Perform point-of-care testing according to policies and procedures when required.
- Collaborate and communicate with physicians about the plan of care and changes to patient status, provide regular progress updates, recommend revisions to the plan of care, and document status and communication in accordance with regulations.
- Coordinate and communicate with multidisciplinary team members and outside agencies to achieve expected patient-care and education outcomes through end of life or discharge, ensuring accurate documentation and coordination of individualized care plans.
- Complete admission paperwork, verify insurance information, enter assessment data into the clinical computer program, and complete patient assessments in a timely manner.
- Provide skilled nursing care within the established plan of care, including taking vital signs, wound care, intravenous care, ostomy care, blood draws, injections, and physical assessment of the patient’s overall medical condition.
- Document visit notes, treatments, and care; follow the physician-ordered visit pattern; and ensure required visits and documentation are timely, accurate, and complete.
- Collaborate with patients, families, and the healthcare team to develop comprehensive plans of care based on individual patient needs.
- Respond to calls from patients, family members, and others, and complete follow-up to address needs, issues, or concerns.
- Maintain constant alertness and work safely.
- Perform other assigned duties.
