  • A current Nursing Assistant License in the State of New Hampshire is required.
  • Basic Life Support authorization is preferred or must be obtained within 90 days of hire.
  • A minimum of 6 months of experience as a Licensed Nursing Assistant is required.
  • Proficiency with Microsoft Office is required.
Responsibilities
  • Provide direct and indirect patient care under the direction of a registered nurse and within the scope of practice of a Licensed Nursing Assistant.
  • Assist patients with activities of daily living, personal care, emotional support, and complex clinical skills under registered nurse direction.
  • Perform or assist with activities of daily living and provide age-specific patient care.
  • Provide basic support to patients and families in response to psychosocial needs and cultural and religious preferences.
  • Perform clinical tasks within the Licensed Nursing Assistant scope of practice and follow applicable policies and procedures.
  • Report observations, symptoms, deviations from normal, and changes in patient condition to the registered nurse.
  • Assist others with selected patient-care activities or tasks as appropriate.
  • Follow infection-prevention and universal-precautions policies.
  • Incorporate safety principles, policies, and procedures into daily practice.
  • Maintain patient confidentiality, privacy, and dignity.
  • Perform routine and emergency phlebotomy.
  • Complete telemetry rhythm analysis and interpretation and post rhythm strips at specified times.
  • Communicate rhythm changes and vital-sign abnormalities to the primary and charge nurses.
  • Track patients who are off the unit for testing and maintain the telemetry alarm-notification log.
  • Ensure communication devices are working properly and are assigned and distributed to nursing staff.
  • Communicate effectively with visitors, callers, physicians, and staff from other departments.
  • Greet visitors, determine the purpose of their visit, and respond or refer them to the appropriate staff member.
  • Update and maintain patient assignment boards, census change sheets, patient lists, and unit communication-device lists.
  • Enter and transcribe orders and data accurately and in a timely manner.
  • Maintain the unit secretary task list and ensure completed items are verified by the registered nurse and entered into appropriate patient-care documents.
  • Assemble admission charts and prepare transfer charts according to required documentation and payer criteria.
  • Disassemble and reorganize charts upon discharge before forwarding them to medical records.
  • Coordinate completion of patient death certificates by contacting appropriate parties.
  • Perform clerical and coordinating activities designated by the unit.
  • Schedule, track, and ensure timely diagnostic testing and obtain insurance authorization for diagnostic testing.
  • Schedule follow-up clinic appointments and coordinate weekly treatment visits for physicians and registered nurses.
  • Review patient charts to ensure necessary information is available before visits.
  • Obtain updated medication lists and coordinate needed services and referrals for follow-up patients.
  • Track missed appointments, notify patients to reschedule, and send certified notices for repeated missed appointments according to protocol.
  • Assist physicians with physical examinations and procedures and prepare required equipment.
  • Stock and clean examination rooms, replace expired items, and manage linen inventory and distribution.
  • Order departmental clinical supplies in the Lawson system, track bulk-store and MDS supplies, and recommend adjustments to par levels.
  • Maintain constant visual observation of patients receiving remote visual monitoring.
  • Participate in handoffs during shift changes or new admissions by communicating pertinent patient information and behavior.
  • Ensure another telesitter is in place before leaving the monitoring station.
  • Verbally redirect patients when necessary and provide explanations without counseling or offering opinions.
Desired Qualifications
  • A high school diploma or equivalent is preferred.
  • Basic Life Support authorization is preferred.
