Requirements
  • Current Registered Nurse licensure in good standing in the state or states of practice, with one year of experience under the license.
  • The candidate must have clinical and communication skills and be passionate about hospice care.
  • The candidate must be able to work collaboratively with nurses, social workers, chaplains, volunteers, and certified nursing assistant leadership.
  • The candidate must be able to work independently with phone, email, and written communication, meetings with healthcare professionals, and an understanding of hospice rules and regulations.
  • The candidate must have time-management skills to manage visits, documentation, meetings, scheduled visits, required meetings, urgent needs, and multiple competing concerns.
  • The candidate must be able to learn an electronic medical records system.
  • The candidate must be willing to drive with reliable transportation, a valid driver's license, and auto insurance across the service area.
Responsibilities
  • Provide care and services to a group of hospice patients in private homes, assisted living facilities, and hospitals.
  • Complete hospice admissions and patient visits as needed.
  • Answer calls from patients, families, and customers.
  • Make nursing visits for symptom management, deaths, and admissions as needed.
  • Advocate for hospice services and Suncrest Hospice.
  • Work after hours on-call for seven nights on and seven nights off.

Interdisciplinary teams coordinate nursing, medical, social-work, spiritual, aide, and bereavement support.
