- Registered Nurse currently licensed in the state where care is provided.
- Must maintain confidentiality of patient information and records.
- Must demonstrate commitment to the hospice philosophy of care.
- Must successfully pass background checks.
- Must have a valid driver's license.
- Must demonstrate strong organizational and self-management skills and good judgment.
- Receive patient assignments and coordinate communication with attending physicians, hospice staff, and other agencies to optimize patient and family care and resource use.
- Maintain regular communication with attending physicians and patient families regarding care.
- Obtain physical, psychological, social, and spiritual patient and family data and incorporate it into the Plan of Care.
- Maintain accurate and current patient records and submit required paperwork in accordance with Medicare and hospice policies and procedures.
- Teach patients and caregivers medication administration, side-effect and adverse-reaction recognition, personal care, oral hygiene, lifting, moving, ambulation, exercise, disaster and safety management, and preparation for events preceding death.
- Inform the Patient Care Manager or triage team of unusual or potentially problematic patient or family issues.
- Attend hospice team meetings and patient conferences as directed by the Patient Care Manager.
- Provide appropriate support at the time of death.
- Provide on-call coverage after hours on weekdays and weekends on a seven-days-on, seven-days-off schedule.
- Develop understanding of hospice philosophy through individualized study.
- Develop proficiency in pain management through reading, in-service training, or seminars.
