Requirements
  • Master's degree in Social Work (MSW).
  • Must possess a valid state-required social work license.
  • Two to five years of related social work experience.
  • Ability to travel if assigned to multiple facilities.
Responsibilities
  • Assess patients' psychosocial status, strengths, and needs as part of a comprehensive patient assessment within an interdisciplinary team.
  • Collaborate with patients and the healthcare team to develop care plans and identify interventions that support rehabilitation and treatment goals.
  • Utilize patient education programs, social work theory, and quality of life measurement instruments to address barriers to treatment.
  • Provide monitoring and interventions to help patients adjust to dialysis and achieve optimal psychosocial status.
  • Offer supportive counseling services to patients within the scope of clinical training and state licensure.
  • Provide educational and goal-directed counseling to patients seeking kidney transplants.
  • Refer patients to community resources such as home health services and vocational rehabilitation to facilitate treatment outcomes.
  • Maintain knowledge of local vocational and educational programs to assist patients in remaining employed or receiving education.
  • Assess patient awareness of advance directives and facilitate discussions regarding advance directive wishes with the healthcare team and family.
  • Participate in discussions regarding patient Do Not Resuscitate (DNR) status to ensure informed decision-making.
  • Adhere to company Social Work Policy and Measuring Patient Physical and Mental Function Policy, including all required documentation.
  • Document all interactions and interventions provided to patients and their families.
  • Participate in monthly Quality Assessment and Improvement (QAI) review meetings and report on indicators such as treatment adherence and quality of life trends.
  • Assess patient knowledge of kidney disease and provide tailored education to address learning barriers.
  • Facilitate the transplant referral process and collaborate with the team on transplant waitlist management.
  • Review patient rights, responsibilities, and grievance procedures with patients and representatives.
  • Interview patients requesting discharge to identify root causes such as relocation or dissatisfaction and share findings with leadership.
  • Act as a liaison with Insurance Coordinators to educate patients on payment options, including federal, state, and commercial insurance.
  • Assist with the interview and hiring process for new personnel if requested by management.
  • Provide training to staff on psychosocial topics and participate in weekly team huddles to discuss urgent patient issues.
