Requirements
  • A Registered Nurse license from the Missouri Board of Nursing is required.
  • At least 2 years of clinical experience and 1 year of experience working with elders or adults with disabilities are required.
  • Experience with customer relationship management systems is required; Salesforce experience is a plus.
  • A valid driver’s license and an insured vehicle for travel within Missouri are required.
  • Understanding of regulatory and provider compliance requirements is required.
  • Policy development and implementation experience is required.
  • Strong communication, interpersonal, problem-solving, decision-making, organizational, multitasking, critical-thinking, and self-motivation abilities are required.
  • Ability to thrive in a fast-paced startup environment is required.
Responsibilities
  • Provide support to caregivers and members and review services according to the Personal Care Services program scope.
  • Respect client and employee rights and privacy, secure protected information, practice ethically, and comply with the agency’s compliance program and privacy policy.
  • Follow the company’s clinical policies and procedures.
  • Assess and regularly reassess client needs, primary care provider summary forms, treatment, and education, and communicate changes to the member’s primary care provider.
  • Develop, implement, evaluate, revise, and provide input on client plans of care, including physician treatment plans and problem lists.
  • Provide care instructions to caregivers according to the plan of care.
  • Complete accurate, timely, complete, concise, and legible client documentation, including written records, reports, and notes, after client visits or contacts.
  • Coordinate client care through communication and documentation with physicians, registered nurses, program directors, other involved disciplines, and community resources.
  • Interpret the agency’s philosophy and objectives to clients, families, and others.
  • Perform other job-related duties as directed by the supervisor.
Desired Qualifications
  • Bilingual Spanish ability is a plus.
  • Salesforce experience is a plus.
