Requirements
  • Master’s or Doctorate degree from accredited Nursing Program for Nurse Practitioner, or Bachelor’s or Master’s degree from accredited Physician Assistant Program for Physician Assistant.
  • State of Wisconsin NP or PA licensure.
  • Active RN and APRN certifications and licenses, including prescriptive authority, in state(s) of practice; Active board certification as a Nurse Practitioner in area of clinical practice (e.g., Family).
  • Active PA license, including prescriptive authority, in state(s) of practice; Active PA board certification.
  • Active DEA license or ability to apply for such license prior to or upon hire.
  • At least two years’ experience in Family Medicine, Primary Care or Internal Medicine.
Responsibilities
  • Float to a variety of clinics; rotate one week at the UW Health Sub-Zero Employer On Site Services, located in Fitchburg, WI and one week at clinics within UW Health including Family Medicine and General Internal Medicine within Madison, WI and the surrounding regional clinics.
  • Perform annual physical exams and help keep patients up to date on health maintenance.
  • Care for the acute and chronic needs of patients from newborn to elderly.
  • While at the Sub-Zero Clinic, you will be the only advanced practice provider in the clinic, requiring the ability to make decisions independently.
