  • Bachelor’s degree in social work, gerontology, psychology, or related field
  • Experience working with older adults
  • Warm and friendly personality
  • Customer service/hospitality experience helpful

  • Proficient documentation skills
  • Ability to interact positively with clients, other community organizations, vendors and other EverTrue staff
  • Desire to seek new and innovative ways to respond to the needs and demands of each client
  • Ability to prioritize calls and maintain appropriate call back time based on volume of calls and clients’ needs
  • Skills necessary to develop a working relationship with community-based organizations and vendors
  • Ability to attend older adult events in the community as a representative of EverTrue as assigned
Responsibilities
  • Builds trusted relationships with each client as assigned by an EverTrue care manager (“Social Worker, MSW”); This will be accomplished through regular visits, phone calls and other communication with each client
  • Listens attentively to the client and their needs, consulting with care manager on how to support client
  • Always demonstrates respect for the client and upholds HIPAA requirements
  • Works with care manager and client on helping clients meet their identified personalized goals
  • Documents each interaction in current documentation system and updates care manager on any client change in condition, situation, or goal
  • Supports clients by understanding the care plan goals
  • Monitors and tracks the number of times they and/or their care manager has contact with each client each week and ensures documentation is up to date and that services are billed accurately
  • Maintains an understanding of the model of service provided; Supports the marketing of the Anywhere Care care management program
  • Assists their care manager coordinate services on behalf of clients that are appropriate; Ensures clients understand cost of services
  • Ensures that clients are provided choice when hiring outside services by reviewing options with each client for them to choose from
  • Collaborates with the EverTrue leadership team to plan events and activities for clients
