Summary:
A Client Advocate is responsible for supporting and working collaboratively with care managers by assisting with client calls, making referrals, providing supportive client visits, and ensuring monthly billing charges are submitted; The goal of this position is to build a trusted relationship with each client and assist with coordination of services to improve the clients’ home, community, and well-being so they may stay in their home for as long as they choose.
Essential Duties and Responsibilities:
1. Builds trusted relationships with each client as assigned by an EverTrue care manager (“Social Worker, MSW”); This will be accomplished through regular visits, phone calls and other communication with each client
2. Listens attentively to the client and their needs, consulting with care manager on how to support client
3. Always demonstrates respect for the client and upholds HIPAA requirements
4. Works with care manager and client on helping clients meet their identified personalized goals
5. Documents each interaction in current documentation system and updates care manager on any client change in condition, situation, or goal
6. Supports clients by understanding the care plan goals
7. Monitors and tracks the number of times they and/or their care manager has contact with each client each week and ensures documentation is up to date and that services are billed accurately
8. Maintains an understanding of the model of service provided; Supports the marketing of the Anywhere Care care management program
9. Assists their care manager coordinate services on behalf of clients that are appropriate; Ensures clients understand cost of services
10. Ensures that clients are provided choice when hiring outside services by reviewing options with each client for them to choose from
11. Collaborates with the EverTrue leadership team to plan events and activities for clients

Bachelor’s degree in social work, gerontology, psychology, or related field
Experience working with older adults
Warm and friendly personality
Customer service/hospitality experience helpful
Acute listening skills
Proficient documentation skills
Ability to interact positively with clients, other community organizations, vendors and other EverTrue staff
Desire to seek new and innovative ways to respond to the needs and demands of each client
Ability to prioritize calls and maintain appropriate call back time based on volume of calls and clients’ needs
Skills necessary to develop a working relationship with community-based organizations and vendors
Ability to attend older adult events in the community as a representative of EverTrue as assigned
Physical Activities and Demands:
Must be able to complete all physical demands of the job, which may include, but not be limited to: the ability to lift and transfer patients, carry supplies, and read 12 point or larger typewritten print. Have corrected vision and hearing within normal range and have manual dexterity with normal range of motion of all extremities. May be requested to work weekends, holidays, and occasional overtime. Must be able to work alone without usual support systems immediately available in acute care settings. Must have the ability to deal with abrupt schedule changes and to maintain objectivity in coping with the stress of working with acute, chronic and terminally ill patients.

Must have personal transportation to commute to clients’ homes and community events at various locations in the St. Louis metropolitan area
A well-lit and ventilated office working area

Minimal exposure to chemicals and hazardous waste
Noise level in work environment is usually quiet

EverTrue Pay & Benefits
Pay grade: B ($47,000.00/year to $75,000.00/year)
