Responsibilities
  • Conducts and documents detailed psycho-social assessments/evaluations to interpret social, environmental, and medical needs of the patient and family
  • Manages length of stay by creating a successful discharge plan and by proactively identifying and mitigating issues and barriers to patient care
  • Provides supportive counseling and advocacy to assist patients and/or families with adjustment associated with illness, hospitalization, and/or alternative care placement
  • Considers options such as palliative care, homecare, and other services that work to keep the patient as healthy as possible in the outpatient setting
  • Monitors risk assessment using available tools and implements discharge interventions, as necessary
  • Facilitates the resolution of issues surrounding patient care in a compassionate manner
  • Negotiates with patient, family, and third party payors for an optimum plan of care for patient and family
  • Intervenes and communicates with appropriate individuals (physician team members and insurance companies) to ensure appropriate utilization of resources, quality care, and care facilitation

Pa. 'Blue Envelope' program helps first responders aid drivers with autism. Jefferson Health's Blue Envelope program is expanding to Luzerne County, giving drivers on the autism spectrum a tool to help make traffic stops safer and less stressful. April 12, 2026 08:00 AM By Gregory Adams Standard-Speaker WILKES-BARRE TOWNSHIP, Pa. - Lehigh Valley Health Network (LVHN), a part of Jefferson Health, officially launched the Blue Envelope program with Luzerne County police departments on Wednesday at the Wilkes-Barre Twp. Firehouse. The program, which has already been enacted by departments in Monroe, Lehigh and Northampton counties, is aimed at helping smooth communication between drivers with special needs and police officers. Under the program, drivers carry a blue envelope in their vehicle. The front of the envelope notes the driver is on the autism spectrum and indicates if that person is verbal or nonverbal. The envelope includes instructions for the driver on what to do during a traffic stop. The back of the envelope has tips for police to help their interaction with the driver go smoothly. Inside the envelope is a sheet that explains the program and provides contact information to medical attention or assistance in interacting with the driver if that were to become necessary. The sheet also explains that autism spectrum disorder (ASD) can impair the driver's ability to communicate and they may have difficulty understanding or responding to questions. Kingston Police Chief Richard Kotchik believes the implementation of the program is an important way to enhance how local departments can serve and protect all residents. "This program strengthens the relationship between law enforcement and the community," Kotchik said. "It demonstrates our commitment to evolving, listening and finding better ways to serve, especially those who may need a little extra support." Erin Ludwig, executive director of community engagement with Jefferson Health, spoke to the audience of local officers and noted having to interact with law enforcement could make anyone anxious but for individuals on the autism spectrum those interactions can be especially challenging. Since the program's launch in Lehigh County in April 2024, more than 10,000 blue envelopes have been distributed and have been made available to over 50 law enforcement agencies. April is also World Autism Month. Kotchik said he was first made aware of the program through other police chiefs in the county. Dallas Twp was the first local agency to implement the blue envelopes last year and after hearing about their success with it was more than happy to introduce it county-wide. "I think it's going to be great for the guys to start learning more about it and I think this is going to be the way to do it," he added. Officers will have literature available to go over about the program as well as trainings on autism and different signs to look for during interactions. Kotchik said he will get the envelopes and literature out to every department in the county for use. The program was birthed through a partnership between LVHN and Salisbury Township., Lehigh County. Ludwig says the feedback has been satisfying. "Its been great to see some of the extensions of the program. It's very interesting to schools and driver education programs, colleges and universities, community partners, and that's what's continued to inspire us, just the continued enthusiasm of being open to creating better understanding between these populations," she remarked. Ludwig said they will continue to respond to community need in terms of expanding the program, noting there are still ample growth opportunities. The resort, which features a 220,000-square foot waterpark, attracts more than 2 million people each year and is a major factor in Poconos tourism. Did you know? Jefferson recently opened its newest neighborhood hospital - LVH-Pocono Creek and Health Center at Pocono Creek - nearby in central Monroe County. "It will enhance our guest resort experience with new sensory elements and training and provide a valuable resource for the community and our team members." Jefferson Health is participating in the development of a new sensory-friendly Calming Room - set to open in summer 2026 - providing a key resource for neurodiverse families to stay close to the fun while enjoying the resort. Additionally, the collaboration will enhance water and sun-safety messaging for guests throughout the resort, host employee wellness activities for Kalahari associates, and on-site participation in Kalahari community offerings throughout the year. Kalahari Resorts, which opened its Tobyhanna Township, Monroe County, resort in 2015, employs about 1,400 people. The resort, visible from Interstate 380, also features an outdoor waterpark and the Gorilla Grove outdoor park area with numerous attractions. Lehigh Valley Hospital receives $250,000 grant to expand health care workforce education

Lehigh Valley Hospital receives $250,000 grant to expand health care workforce education. The region is getting another boost towards health care workforce development. Lehigh Valley Hospital has been awarded a $250,000 Schools-to-Work Program (STWP) grant through the Pennsylvania Department of Labor & Industry to expand its workforce development initiative, Health Care Futures, in partnership with the Allentown School District's Bridgeview Academy of Health, Science, Innovation and Technology. With the funding, Health Care Futures will expand career exposure and job placement support to Bridgeview while developing and implementing a comprehensive allied health curriculum for students in grades 9-12. "This investment is about unlocking new opportunities for our students," said State Sen. Nick Miller, D-Lehigh/Northampton. "Health Care Futures doesn't just prepare Bridgeview students for graduation. It connects them to family-sustaining careers in health care, giving them hands-on experience that sets them apart in the real world. With this $250,000 investment secured by my office, we are equipping students with the skills, experience, and confidence to build lasting careers, strengthen our local workforce, and serve our community for years to come." The Department of Labor & Industry awards STWP funding to employers, schools, and community organizations to create, launch, and sustain registered pre-apprenticeship career pathways. These programs are designed to bridge the gap between high school or college graduation and successful entry into the workforce. "The Schools-to-Work grant represents a transformative investment in the students of the Allentown School District," said Carol Birks, Allentown School District superintendent. "By strengthening our partnership with Lehigh Valley Hospital, we are ensuring that our students don't just learn about the medical field in a classroom, they are able to experience it firsthand. This collaboration provides a direct pathway to high-demand careers, empowering our youth with the technical skills and professional mentorship needed to lead as the next generation of healthcare professionals." The announcement follows ASD Bridgeview's January groundbreaking for the new Family and Community Resource Center, which will be the new centralize enrollment support, expand access to coordinated health and wellness services, and offer workforce development resources to the community.
