Requirements
  • A minimum degree in social work, psychology, or gerontology is preferred.
  • At least two years of experience in social service delivery with elderly and/or family populations is required.
  • Demonstrated working knowledge of supportive services and other resources in the area served by the project is required.
  • Demonstrated ability to advocate, organize, problem-solve, and provide results for residents is required.
  • Good communication, comprehension, and interpersonal skills are required.
  • Working knowledge of Windows, Microsoft Office Suite, and resident software data systems such as AASC Online, Care Guide, RealPage, or similar systems is required.
Responsibilities
  • Initiate and complete assessments by interviewing residents, families, and others to evaluate current conditions and identify current or potential needs and services.
  • Provide general case management and referral services.
  • Coordinate communication among residents, family members, medical staff, administrative staff, and regulatory agencies.
  • Coordinate services for optimal living, including home care services that enable independent living and aging in place.
  • Coordinate counseling, occasional nursing visits, preventive health screening and wellness services, legal advocacy, Meals on Wheels services, and transportation.
  • Establish and coordinate volunteer and support programs.
  • Maintain knowledge of community services and resources available to residents, establish relationships with local service agencies and providers, and monitor quality and effectiveness.
  • Assist residents with building informal support networks involving other residents, family, and friends.
  • Explain policies, procedures, and services to residents.
  • Keep current with developments in the field through literature, colleagues, continuing education, professional organizations, and conferences.
  • Provide training to residents, volunteers, and staff on service availability, application procedures, resident rights, health, safety, fitness, and aging-related issues.
  • Provide Administration with regular status reports.
  • Identify and report resident violations to appropriate authorities.
  • Maintain accurate, complete, and updated client, program, and activity files in computer and hard-copy formats.
  • Attend and participate in appropriate meetings and training opportunities.
  • Follow Presbyterian Villages policies and procedures, safety policies and procedures, and applicable federal, state, and local regulations.
  • Perform other duties as assigned.
