Requirements
  • Bachelor of Science Degree in Nursing
  • compact unrestricted Registered Nurse (RN) license
  • 5 years of nursing experience
  • 3 years in solid tumor oncology nursing
  • Oncology-related certifications such as OCN, AOCN, AOCNS, or CCM
Responsibilities
  • Conduct comprehensive clinical assessments
  • Oversee member health
  • Facilitate end-of-life care discussions
  • Engage with members and address clinical issues
  • Manage escalations
  • Familiarize with Thyme Care systems and tools
  • Collaborate with Nurse leaders and Medical Directors
  • Establish trusting relationships with members
  • Adhere to Care Team policies and procedures
  • Support members throughout the oncology care continuum
  • Identify and address member needs promptly
  • Participate in case conferences
  • Foster partnerships with payers and providers
  • Collaborate with non-clinical Care Team members
  • Be available for urgent clinical escalations
