Requirements
  • Current Registered Nurse licensure in good standing in the state or states of practice.
  • Ability to collaborate in a team environment with nurses, social workers, chaplains, volunteers, and certified nursing assistant leadership.
  • Ability to work one-to-one with patients while receiving support from a patient care team.
  • Ability to work independently with phone, email, and written communication; meetings with healthcare professionals; and knowledge of hospice rules and regulations.
  • Time management skills to manage visits, documentation, meetings, scheduled visits, required meetings, and urgent needs by prioritizing and triaging multiple concerns.
  • Ability to learn an electronic medical records system.
  • Willingness to drive with reliable transportation, a valid driver's license, and auto insurance to travel across the service area.
Responsibilities
  • Provide care and services for a group of hospice patients in their residences, including private homes, skilled nursing facilities, and assisted living facilities.
  • Contribute to the interdisciplinary team by providing expertise regarding psychosocial assessments, interventions, and community resources.
  • Work in a Case Management, Admissions, or Resource role.
  • Visit patients where they reside.
  • Manage visits, documentation, meetings, scheduled visits, and urgent patient needs by prioritizing and triaging multiple concerns.
