Requirements

Responsibilities
  • Demonstrate understanding and implementation of regulatory guidelines; audit residents' medical records and maintain required audit documentation.
  • Close medical records following a resident's hospitalization or permanent discharge.
  • Perform medical record procedures under the direction of the Unit Manager.
  • Protect, safeguard, and maintain the confidentiality of resident information.
  • Assist Medical Records with preparing admission and discharge summary forms.
  • Type organizational lists and perform other secretarial functions for the nursing unit.
  • Distribute mail to residents and nursing-unit staff.
  • Answer call-light signals and direct or assist as appropriate.
  • Coordinate preparation of forms and documents needed for Medical Records, Treatment Books, Resident Care Planning meetings, and MDS assessments, including aide assignment sheets, ROM/Ambulation Records, and skin and behavior records.
  • Coordinate collection of information from all disciplines for the Inter-disciplinary Discharge Summary and send it to the appropriate destination in a timely manner.
  • Schedule resident appointments for consultations and follow-up visits, including podiatry, dental consultant appointments, and influenza vaccinations when applicable.
  • Notify attending physicians of required visit due dates.
  • Manage inventory and requisition supplies for the nursing unit, including nursing treatment supplies and printed forms, as assigned.
  • Maintain lab report schedules for admission, routine, annual, or special resident requirements; complete appropriate forms and schedule appointments for lab draws.
  • Assist with obtaining statistics for nursing monthly reports and annual surveys by the state health department.
  • Photograph residents upon admission and place the picture in the Medication Book.
  • Assist with obtaining room labels for resident rooms.
  • Assist with transporting residents and passing meals as necessary and directed.
  • Obtain transportation approvals and schedule van transportation.
  • Attend facility meetings, staff meetings, and required or preferred in-services.
Desired Qualifications
  • A high school diploma or equivalent is preferred.
  • Knowledge of medical records processing is preferred.

Elderwood partners with Trocaire College to advance nursing careers of current employees. Scholarship program provides both financial support and mentorship. Trocaire College in partnership with Elderwood, announced the launch of the Elderwood-Trocaire iCARE Career Pathway Scholarship. This initiative represents a $150,000 annual investment by Elderwood to support up to ten employees at a time as they pursue a practical nursing (LPN), registered nursing (RN), or bachelor's degree in nursing (BSN) at Trocaire. "This new scholarship program is another opportunity for Trocaire and Elderwood to work together to help strengthen the local healthcare workforce," said Dr. Bassam Deeb, president, Trocaire College. "The demand for qualified nurses locally continues, and with Elderwood's commitment to investing in its employees, this program provides financial support, mentorship, and a clear path forward for those looking to advance in their healthcare careers." Beyond financial support, the partnership between Trocaire and Elderwood provides mentorship and guidance from experienced clinical and operational leaders, as well as helping the participants successfully navigate their education and transition into advanced nursing roles. Full-time and part-time academic program opportunities are available. Employees must continue working at least 15 hours per week while enrolled at Trocaire and commit to one-year of employment with Elderwood upon program completion. "We see this partnership as more than a scholarship, it's an investment in people who are already committed to caring for others," said Dr. Jeffrey Rubin, co-CEO of Post Acute Partners, owner of Elderwood. Elderwood acquires Weinberg Campus for $47M

Employees, whose one-year contract expired last month, have only received one-year extensions since Elderwood announced plans in 2017 to purchase the Weinberg Campus for $47 million.
