Requirements
  • A Bachelor of Science in Nursing or Master’s degree in Nursing is required.
  • Current California Registered Nurse licensure is required.
  • Three years of experience as a registered nurse in an ambulatory/clinic or case management setting is required.
  • The role requires knowledge of non-coverage and denial notices, including Medicare and HSAG guidelines.
  • The role requires knowledge of regulations, standards, and legislation related to the continuum of care and patient transitions.
  • The role requires knowledge of healthcare delivery systems and payer prior-approval requirements.
  • The role requires clinical and critical-thinking skills and the ability to establish and execute complex plans of care.
  • The role requires written and verbal communication skills and the ability to communicate accurately with patients, healthcare professionals, and staff.
Responsibilities
  • Establish rapport with patients and families, understand their needs and expectations, and help establish realistic mutual goals while incorporating relevant cultural factors.
  • Collaborate with physicians, patients, and interdisciplinary teams to establish comprehensive plans of care and address clinical milestones.
  • Communicate plans of care and changes to patients, families, physicians, and healthcare team members, facilitating communication among them.
  • Gather information from relevant sources to evaluate whether plans of care are effective, safe, timely, accurate, and cost-effective.
  • Organize, integrate, and evaluate plans of care and progress toward desired outcomes, modifying plans as patient and family needs change.
  • Identify and resolve clinical and operational roadblocks, identify alternatives, and communicate decisions according to regulatory guidelines and hospital policies.
  • Assist with developing and implementing care performance-improvement plans based on patterns and trends identified through data and observations.
  • Document care-management assessments and interventions.
  • Update care plans as patients progress toward clinical milestones and keep the care team, patient, and family informed.
  • Refer patients to social workers or discharge planners for complex psychosocial and discharge-planning issues and consult interdisciplinary team members to support safe discharge.
  • Communicate continuity-of-care information to post-acute providers through discharge paperwork or telephone communication.
  • Develop and maintain relationships with community health resources and provide resource information to patients.
  • Help patients obtain a primary care physician, affordable discharge medications, and other necessary resources.
  • Ensure appropriate follow-up appointments are scheduled and patients understand post-discharge activities.
  • Monitor and manage clinical outcomes and intervene when issues may affect quality.
  • Perform proactive outreach to high-risk patients, including patients with frequent emergency-department visits, hospitalizations, or chronic conditions.
  • Coordinate transitions of care after emergency-department visits, inpatient stays, and skilled-nursing-facility discharges to support timely follow-up and prevent readmissions.
  • Collaborate with primary care providers, specialists, and interdisciplinary teams to align care plans and close care gaps.
  • Facilitate referrals to internal programs and external community resources, including social services, behavioral health, and pharmacy support.
  • Educate patients and caregivers about disease management, medication adherence, and navigating the healthcare system.
  • Identify and escalate clinical concerns or barriers to care to appropriate providers or leadership.
  • Participate in interdisciplinary case conferences, team huddles, and quality committees.
  • Use data, registries, and dashboards to prioritize outreach and measure intervention effectiveness.
  • Assist with advance-care-planning discussions and long-term-care planning when appropriate.
  • Participate actively in professional governance.
  • Perform other duties as assigned.
