Requirements
  • Graduate of an accredited School of Vocational/Practical Nursing.
  • Hold a current state license as a Licensed Vocational/Practical Nurse, or a qualifying multi-state license issued under the National Nurse Licensure Compact.
  • Hold a current driver's license.
  • Have two years of experience as a Licensed Vocational/Practical Nurse in a clinical care setting.
  • Demonstrate proficiency in nursing skills according to generally accepted standards of practice.
  • Have reliable transportation and valid, current automobile liability insurance.
  • Be able to work in patients' homes in various conditions.
  • Be able to work a flexible schedule and travel locally.
  • Be able to lift up to 50 pounds and move patients.
  • Be able to work under stressful conditions to meet deadlines and patient needs.
  • Be able to make quick decisions and acquire necessary resources.
  • Be able to meet individualized patient and family psychosocial needs.
Responsibilities
  • Provide nursing care to patients of all ages in their homes under the supervision of a registered nurse.
  • Coordinate care with the interdisciplinary group, patient and family, and referring agency.
  • Assist in developing and implementing patient care plans.
  • Monitor patient health and report changes to the supervising registered nurse.
  • Administer medications and treatments as prescribed.
  • Educate patients and their families about health care needs, conditions, and options.
  • Maintain accurate and timely documentation of patient care and progress.
Desired Qualifications
  • Home health experience.
  • Experience with HCHB.
