- A high school diploma or GED is required.
- At least one year of experience in a healthcare setting is required; volunteer and internship time is acceptable.
- Knowledge of Microsoft Office products and general computer literacy is required.
- Strong interpersonal skills, willingness to learn, and the ability to work in a fast-paced and changing environment are required.
- Excellent written and oral communication skills are required.
- Detail orientation, good organizational skills, and the ability to multitask are required.
- The ability to work independently is required.
- Assist with facilitating successful discharge plans that support length-of-stay reduction and patient flow.
- Locate resources and rapidly respond to requests from Care Management, Social Work, and unit staff.
- Provide a high level of customer service to patients when preparing for discharge.
- Make regular rounds of all patients to ensure assigned treatment plans are being implemented as determined by the treatment team.
- Work closely with Care Management and Social Work to ensure proper and timely patient discharges.
- Ensure discharge-plan details are communicated to the interdisciplinary treatment team.
- A bachelor's degree is preferred.
