Requirements
  • A high school diploma or equivalent is preferred, or substantial work experience.
  • Past home care coordination experience is highly desirable, and previous customer service experience is preferred.
  • Excellent telephone and customer service skills are required.
  • Relationship-building skills are required.
  • Strong organizational skills are required.
  • Strong basic computer and office skills, specifically with Microsoft Office, are required.
  • The ability to plan work and meet required deadlines is required.
  • The ability to work in a fast-paced environment is required.
  • Basic math skills are required.
  • The ability to learn insurance terminology related to authorizations, claims, and payments is required.
  • The ability to perform multiple tasks efficiently and accurately, proactively identify problems, and resolve issues with minimal supervision is required.
Responsibilities
  • Coordinate and schedule home care services to implement the plan of care established by the interdisciplinary team.
  • Work collaboratively with home care nurses, other staff, vendors, participants, and families to support high-quality care and service and positive internal and external customer relations.
  • Communicate with home care agencies and other vendors to coordinate initial orders, changes, cancellations, and resumption of services.
  • Email or fax open case needs to home care agencies as required.
  • Use established systems and procedures to identify problem cases and alert management when additional intervention is needed.
  • Follow up on all non-assigned cases until services are secured.
  • Document vendor authorizations and service coordination efforts in the electronic medical record and other assigned systems.
  • Document participant-related telephone calls in the electronic medical record.
  • Facilitate timely delivery of service authorizations to vendors.
  • Maintain an accurate record of participants’ home care services in a designated format.
  • Use established systems to track assigned data and prepare requested reports.
  • Conduct periodic audits of claims paid to ensure services were authorized by the interdisciplinary team and facilitate resolution of identified issues.
  • Perform photocopying, faxing, and other clerical duties as required.
  • Perform other duties assigned by the Operations Manager and other leadership staff.
